Activity Description: Participation in a Perioperative Surgical Home that provides a patient-centered, physician-led, interdisciplinary, team-based system of coordinated patient care from pre-procedure assessment through the acute care episode, recovery, and post-acute care. The clinician must perform one or more of:
- Coordinate with care managers or navigators in the preoperative clinic to plan and implement a comprehensive post-discharge plan of care;
- Deploy perioperative clinic and care processes to reduce post-operative visits to emergency rooms;
- Implement evidence-informed practices and standardize care across the entire spectrum of surgical patients; or
- Implement processes to ensure effective communication and education of patients' post-discharge instructions.
Validation documentation: At least one of: documented conversations with care managers or navigators; documentation of evidence-informed perioperative care processes implemented to standardize care (workflow diagrams, written policies); documented processes for communicating discharge instructions accounting for literacy level, language preference, and functional impairment.
Remember the ground rules: generally at least 90 consecutive days unless the activity specifies otherwise. For group attestation, at least 50% of the clinicians must perform the same activity, but their periods do not need to overlap. Retain documentation for six years after submission.
